
What happens when major US corporations want to improve patient-physician relations to create a more effective and efficient model of healthcare delivery? The Patient-Centered Primary Care Collaborative (PCPCC) has become one of the major developers and advocates of the Patient-Centered Medical Home model in America.
The Patient-Centered Medical Home model is founded on the patient and physician relationship. The physician coordinates all aspects of preventive medicine, acute and chronic needs of patients, and utilization of the best available evidence and appropriate technology to achieve successful outcomes. The ultimate goal is to make sure procedures are relevant and performed efficiently.
