Small Practice Tools

The WAFP understands the difficulty family physicians face when trying to divide their time between seeing patients, running their small practice and spending quality time with family. This page is designed to provide the small practice family physician with resources on how to begin the implementation of the components of PCMH in a small practice setting. These tools and strategies do not require an integrated deluge system, EHR or other big system resources. These strategies and tools can easily be undertaken by small practices and will help your practice move towards becoming a recognized PCMH.
